To change your name on your account you'll need to download, fill and courier us physical copies of the forms and documents as mentioned below.

  1. Account Details Modification Request Form 

  2. Annexure B

  3. KYC Application form

Along with the forms, you'll also need to send the following:

1) Self-attested PAN card copy with the new modified name  (mandatory)

2) Gazette Copy OR Marriage Certificate 

3) Any one of the address proof documents with a new modified name, mentioned below. Should be (self-attested):

- Photocopy of Valid Passport (First and last page)

- Photocopy of Voters Identity card

- Photocopy of Valid Driving Licence (Date of expiry also to be attached)

- Photocopy of the Aadhaar Card.


Kindly fill up these details in the account modification form:

1  Upstox Client ID

2  Basic details-

  1. Mention the name as per the Upstox records.

  2. PAN card

3  Photo with cross signature


Kindly fill up these details in the KYC form:

  1. Name(as per IT).

  2. DOB, “As per Pan”

  3. Pan Number

  4. Marital status is mandatory.

  5. Father/Spouse Name.

  6. Mention the address as per address proof and address type is mandatory.

  7. Photo with cross signature (Signature as per Upstox records)

  8. Client signature, Place and Date required in Declaration part.

  9. Use Black OR Blue Pen.

  10. Avoid use of whitener and any kind of Cancellation/Correction.

  11. Residential Status.

  12. Gender

  13. Citizenship

  14. Mobile and Email ID.
